The Dorchester County Health Department can issue official birth certificates with a raised seal for anyone born in the State of Maryland from 1940 to the present. Birth certificates can only be issued to the person or the parent named on the certificate. Legal guardians must present original Court documents. For individuals born before 1940, the application can be faxed to the Division of Vital Records and the birth certificate will be mailed to the individual's home address by the Division of Vital Records.
The Dorchester County Health Department issues death certificates to funeral directors or other individuals who are entitled. Certificates for deaths that occurred on or after January 1, 2015, can be purchased at this Health Department. Certificates for deaths that occurred from 1969 to the present can be purchased from the Division of Vital Records. For copies of records for deaths that occurred prior to 1969, contact the Maryland State Archives at 1-800-235-4045.
