DORCHESTER COUNTY SCHOOL BASED WELLNESS CENTERS
The Dorchester County Wellness Center Program is dedicated to promoting accessible, quality school-based primary health care for children and youth. School-based health care is built upon mutual respect and collaboration between school personnel and the health providers to promote the health and educational success of school-aged children.
Goals of the Program
To promote overall positive health behaviors and to encourage primary prevention and wellness.
To provide social and mental health counseling; as well as prevention services
To improve the access to preventative and primary care
To increase health knowledge and to enhance positive decision- making skills
To support educational goals
To provide health care in conjunction with primary care physicians
Services Provided
Ø Physical Examinations
Ø Health Assessments
Ø Mental Health assessments and consultation to families regarding mental health issues
Ø Ongoing mental health treatment including individual, group, and family therapy
Ø Illness, injury evaluation and treatment
Ø Referrals for specialty care
Ø Health education

How Do We Care for Your Child?
We involve you
We keep all records confidential according to state law
We notify primary care physicians of any treatment and recommended follow-up
We prescribe and/or dispense medications and monitor the student’s response to the medications
We make referrals and coordinate outside services

Licensed Social Workers
Physician Consultants
Office Secretaries
Helping our students to succeed in school
Enrollment
All students and staff are eligible to receive these services and may enroll as follows:
1. A student registration/history form must be completed.
2. Parents/Guardians must sign the consent to treat form.
3. Families are not billed for the services, but we are required to bill medical insurance.
